Refer a patient

We are delighted to support health professionals by accepting direct patient referrals.

Our nurses are qualified and experienced health professionals, specialising in the treatment of Asthma and COPD.

Sometimes patients need a more tailored approach. If you have a patient with particular respiratory challenges, our support could be invaluable in helping you identify the best treatment plan. Thanks to the support of our funders, donors and partners, we offer free nurse appointments as a complementary service to assist GP clinics and hospitals

Your patient is able to self-refer to us using the book a nurse appointment form. If your patient needs assistance to book with us or you want to provide us with a full medical background, you can fill out the form below and one of our Asthma Nurses will be in touch your patient shortly. With the permission of the patient, we will forward all test results and clinical notes back to you.

If you are interested in upskilling yourself and your team on Asthma and COPD care, you might be interested in taking part in one of our C.A.R.E Courses for health professionals.

FAQs

How long will this form take?

This form should take no longer than 3 to 5 minutes to complete.

Is my patient eligible for a nurse consultation?

Asthma New Zealand offers nurse consultations primarily in the Waitemata, Auckland Central, and Wellington (Capital & Coast) regions. If your patient resides outside these areas, please contact us directly to explore available options.

Are there any costs associated with the nurse services?

Our initial in-person visit and two follow-up sessions or calls are provided free thanks to the generous support of our funders, donors and partners.

What should my patient prepare for their appointment?

During their session, the nurse will assess their current treatment, provide personalised information on managing asthma or COPD, and demonstrate the proper use of respiratory devices like inhalers and spacers. It’s helpful to have their current medications and any questions they may have ready for discussion. Any information you provide in this form regarding their medical background and lifestyle would be very useful.

What if they need to change or cancel an appointment?

If they need to reschedule or cancel their appointment, please call us or send us an email as soon as you can so we can gift this appointment to another patient: reception@asthma.org.nz.
